Mediconsult’s Saga Home Care System implemented in the wellbeing services county of North Ostrobothnia

On December 9, 2025, the Wellbeing Services County of North Ostrobothnia (Pohde) began using the Saga Home care, communal living, and elderly services system, delivered by Mediconsult, in the areas of Hailuoto, Tyrnävä, Kalajoki, and Merijärvi. The system will be rolled out across the entire wellbeing services county during the early months of next year.

 

 

Saga Replaces Legacy Home Care Systems

Saga Home Care replaces the older home care systems previously transferred to Pohde from municipalities and joint municipal authorities. This transition supports the harmonization of home care practices and facilitates shared use of resources. Moving from multiple systems to a single solution also brings significant cost savings for the wellbeing services county.

Comprehensive Functionality for Home Care and Health Services

The Saga Home Care system covers essential social and healthcare functionalities needed in home care, communal living, and home nursing. It is used for daily client data management, mobile documentation, and operational planning and optimization for home care services.

Supporting Professionals in Daily Work

Through Saga Home Care, Pohde’s professionals have real-time, role-based access to all necessary client and patient information. They can record client notes and, via the Kanta integration, view up-to-date information from the national Kanta services. The browser-based solution is accessible anytime and anywhere, including on smartphones and tablets for mobile home care work.

Enhancing Planning and Resource Management

Saga’s operational management tools enable home care coordinators to efficiently manage events and staff resources. The optimization feature allows automatic planning of client visits. The map function displays the location, the employee’s daily route, travel mode, and estimated travel time.

Integrating Service Organization and Delivery

Saga Home Care is part of the Mediconsult Saga system, providing Pohde’s social services with a unified solution for organizing and delivering services. Information from service needs assessments and planning in social care flows seamlessly to the implementation side in home care.

Similarly, professionals responsible for organizing services can view real-time, relevant data about the client and the delivery of their services.
